Grant here. We see so many trends come and go these days, that it's honestly hard to keep up. Between extensions for every part of your body, plastic surgery up the wazoo, and injecting every surface known to man, altering your appearance is honestly just the norm. But there is one growing trend that does have a lot of people stopping in their tracks and questioning if we've gone too far. Let’s break it down.
Over the past few years, a cosmetic procedure called buccal fat removal has quietly become one of the most talked-about aesthetic trends among celebrities. The surgery removes fat pads from the cheeks, creating sharper cheekbones and a more sculpted, angular face.
It can be said that the "look" of buccal fat removal photographs well. It can give that kind of dramatic, angular face shape that you see walking down the runways. But it's also really harsh, and frankly, in real life it's making a lot of people, namely women, look far older and gaunt.
Celebrities getting buccal fat removal isn't new, and we've seen plenty of "transformations" where celebs step out with a suddenly thinner and more sculpted face.
However, the reaction to buccal fat removal has reached an all-time high as actress Margot Robbie recently stepped out on the red carpet, looking far more gaunt than usual.

I mean, she looks like she's aged about 15 years and gone through five rounds of rehab when you compare the most recent photo on the left to the slightly older one on the right.
Margot has clearly gone under a dramatic transformation, and while it's not 100% confirmed her new look is due to buccal fat removal, top plastic surgeons theorize that she's definitely had something done.

"Margot Robbie looks as though she has had either buccal fat removal, cheek enhancement with a small cheek implant, or both," Dr. Calvert, who hosts the Beverly Hills Plastic Surgery Podcast, explained.
Buccal fat removal, also known as a bichectomy, is a cosmetic procedure that permanently removes fat pads from the cheeks to create a slimmer, more sculpted, and defined lower face.
The doc says that some of Robbie's hollow-faced look could be due to what appears to be recent, rapid weight loss.
"However, the sharpness of the cheekbones leads me to believe there has been some type of surgical intervention," he revealed.
I mean, the big question here is "why?" Why on earth are so many beautiful women like Margot Robbie taking their faces and suddenly making them look older, more haggard, and gaunt?
Well, the answer is trends, and as it turns out, the famous "heroin chic" look from the 90s is making a big comeback.
Calvert, who gave Dr. Terry Dubrow a rhinoplasty on Season 3 of The Real Housewives of Orange County, said Robbie's haunting new appearance is something that's actually on trend at the moment.
He points out that the "MAGA Makeover" appearance of overly filled-in facial features has backfired into the exact opposite look.
"As a Beverly Hills Plastic surgeon for the last 20 years, I've seen this look come and go a few times," Dr. Calvert says about the painfully thin and angular faces now covering Hollywood red carpets.
"The filled look is out, and this gaunt, thinner look is making a comeback," he notes.

What we're really seeing here with Margot Robbie and all of these other celebs going for the more gaunt look is not just a trend really. It's seeing just how quickly and extremely the pendulum swings nowadays when it comes to beauty standards.
For years the dominant look in Hollywood was fullness. Lips were filled, cheeks were plumped, and faces were sculpted to appear rounder and softer. It was often mocked as “pillow face,” but regardless, the look dominated red carpets and Instagram feeds for nearly a decade.
But now the pendulum has swung hard in the opposite direction.
Suddenly the goal isn’t volume. It’s sharpness. Everyone is adopting the Kate Moss look with hollow cheeks, razor cheekbones, and dramatically thinner faces. This is now the new aesthetic of "high fashion."
It's also more jarring to see because now so many trends are forming through dramatic facial transformations. Back in the day trends were more about clothing, hair, and makeup. But in the year 2026, with syringes available at your fingertips, changing your entire facial structure has never been easier.
But the main problem with chasing beauty trends through surgery is the simple fact that trends change.
And unlike makeup or hairstyles, the human face doesn’t reset when the aesthetic cycle flips again. Which is why critics argue that many celebrities chasing the gaunt, angular look today may eventually find themselves dealing with the long-term consequences of a trend that was never meant to last.
